REGISTER ON OR BEFORE APRIL 15, 2008

Welcome to registration for Brain Tumor Action Week in Washington, DC. Each person who wishes to participate needs to register in order to attend the education day on Monday, May 5th and to get scheduled appointments with Congresspersons on Tuesday, May 6th.

Appointments will be made with your Senators and your Representative based on your zip code (we need the entire Zip + 4 code). Please note that you might have appointments with your Senators along with other folks who are from your same state – when we make the appointments, we have to consolidate them. If you wish to visit your elected officials with someone else (who may or may not be from your state and district), there is a spot on the form for entering that person’s name (note – that person must also register). Also, please check the box below if you need an extra amount of time for getting from one place to another, so we can try to space your visits out a little more.
